ENTERTAINMENT AGREEMENT
THE PARTIES: This agreement is for mobile disk jockey entertainment services for the event described below, between
the undersigned Client and DJ Jonathan Jessie.
JOB DESCRIPTION: DJ Jonathan Jessie agrees to furnish services to the following Client and specifications.

OPTIONS INCLUDED IN AGREEMENT: _______________________________________________________________________
PAYMENT TERMS: Total fee agreed upon the Client and DJ Jonathan Jessie is _________ . The total remaining amount
is due at the time of the event unless the payment plan was previously arranged, and may be made by cash or check. Checks
can be made out to DJ Jonathan Jessie. There will be a $ 20.00 fee for all returned checks. Gratuities given to a Disk Jockey
at any event are made at the sole discretion of the Client.
CANCELLATION: This Agreement cannot be canceled or modified except in writing by either the Client or DJ Jonathan Jessie..
If Cancellation is initiated by Client less that 60 days prior to event, then the deposit will be forfeited. For cancellations
less that 30 days prior to event, Client is responsible for the total fee or may forfeit deposit only by signing a new Entertainment
Agreement with DJ Jonathan Jessie within two weeks of cancellation for a substitute agreement performed in the following 6
months. If Client has utilized DJ Jonathan Jessie’s party planning services, then the deposit will be returned. Rescheduling
for events canceled due to inclement weather shall be accommodated whenever possible. Rescheduled events are subject to availability.
OVERTIME COST: When feasible, Client request for extended playing time during event will be accommodated Overtime requested
may be made in half-hour increments at a cost of _______$0.00______ per half-hour. Payment for overtime is due on the date
of the event, and may be made with the cash or checks.
Checks can be made out to DJ Jonathan Jessie.
PROVISIONS: The Client shall ensure that: (1) performance facility provides DJ with a sturdy covered table approximately
2X8 feet in an area of 50 feet from a 110 volt outlet (3 prong grounded); (2) facility is open at least 2 hours prior to scheduled
start time; (3) facility meets all federal and state safety regulations. (4) reasonable steps will be taken to protect DJ
Jonathan Jessie’s equipment, personnel, and music, and crowd control will be provided if warranted; (5) for outdoor
performances, shelter is provided that completely covers and protects DJ Jonathan Jessie’s equipment from adverse weather
conditions.
Clients accepts full responsibility and is liable for any damages, injuries, or delays that occur as a result of failure
to comply with these provisions. In the event of circumstances deemed by DJ to present a real or implied injury or harm to
DJ, equipment, or recordings, then DJ reserves the threatening situation. DJ further reserves the rights to deny any guest
access to recordings or equipment. In the unlikely event the DJ’s performance is delayed, liability is limited to providing
Client with performance time equal to time lacking.
